    Ajentik is a Singapore-based AI company building Elderwise, the care coordination platform that bridges hospital to home. Selected as a Harvard Top 10 Venture and incubated at NUS Enterprise, we are on a mission to ensure no family feels lost after hospital discharge.

    To eliminate the communication gap between hospital discharge and home recovery. Every year, millions of elderly patients leave the hospital with complex care plans, only to feel abandoned once they get home. Families struggle to manage medications, follow-up appointments, and recovery protocols without support. We exist to change that.

    We build AI-powered care coordination tools that keep families informed, caregivers supported, and care teams connected throughout the entire care journey.

    Our Journey

    Ajentik was born from a simple but urgent observation: families navigating elder care in Southeast Asia face fragmented systems, language barriers, and a chronic shortage of coordination tools. The idea took root in early 2020, and active platform development began in September 2023.

    In late 2024, Ajentik was incorporated in Singapore and accepted into the National University of Singapore (NUS) Enterprise incubation program. In early 2025, the company was selected as a Top 10 Venture by the Harvard T.H. Chan School of Public Health HSI Lab, and secured its first enterprise client.

    Today, Ajentik is building the AI infrastructure for healthcare coordination in Southeast Asia. Our platform integrates with Singapore's major healthcare clusters, supports four languages, and is designed from the ground up for HIPAA and PDPA compliance. We are not just building technology; we are building the bridge that keeps families connected to the care their loved ones need.

Under his leadership, Ajentik was selected for Singapore's National GRIP 2025 accelerator and named a Harvard HSI Lab Top 10 venture.
